Job Summary:
The Behavioral Health (BH) Therapist is a member of the Behavioral Health (BH) team that includes Integrated Social Workers, BH therapists, an Addictions Recovery therapists, and a psychiatrist. They support the mission and values of Maple City Health Care Center (MCHCC) by fostering a healthy community, focused on the mental health aspects of care. The work includes two roles: ongoing behavioral health therapy and Integrated BH in cross-disciplinary teams to provide behavioral health support to patients.
Salary Range: $26.92-35.57
Supervisory Responsibilities:
Duties and Responsibilities:
Ongoing BH Therapy
- Develops clinical service plans for patients
- Coordinates collaborative, integrated treatment plans shared with the care team
- Provides ongoing therapy to patients
- Participates in regularly scheduled case consultation with other Behavioral Health professionals
- Maintains appropriate and timely medical records for services rendered as a staff member at MCHCC
- Consults with care teams to provide support to providers on particular patients
- Implements, coordinates and hosts groups as needed
- Completes other duties that support our shared work, as assigned
Integrated BH
- Collaborates closely with the integrated Social Worker to provide mental health triage, consultation and support for patients who are scheduled or calling in
- Supports the work of a cross-disciplinary care team to assess psychosocial assets and needs of patients
- Uses a variety of assessment tools to help address the mental health concerns of patients
- Addresses patient behavioral health concerns as they arise in medical appointments
- Coordinates referrals to ongoing BH therapy

Required Skills and Abilities:
- Ability to work both independently and collaboratively

- Bilingual: Spanish/English preferred
- Effective interpersonal skills
Education and Experience:
- Master's level training and licensure required
- LCSW, HSPP, LMCH, LMFT, or LCAC preferred
- Cross-cultural training and experience to effectively function with the MCHCC patient population
- Experience working with all age ranges preferred
- Knowledge and/or training in trauma-informed care preferred; training supported
- Motivational Interviewing training preferred; training supported
- BLS required
Physical Requirements:
- Ability to perform prolonged periods of time in front of a computer
